GSSC Sport & Activity Camp - Outdoor Activity Consent Form

I hereby give permission for my child to participate in the following activities during the Summer Sport and Activity Camp - Activities within Bellahouston Park. I authorise the club officials to provide emergency treatment to any injuries or illness to my child. In such a situation the club will endeavour to contact the parents / guardians concerned to inform them of the situation.

Photographic & Video Consent

Do you grant Glasgow Ski & Snowboard Centre permission to use photos and/or video recordings of you taken during club events for use in club publications (such as newsletters, the website, and social media)?

Declaration

We assume that all risks associated with participation in the above activities, including but not limited to falls, collisions, injury and other reasonable risk associated with the sport. All such risks to my child are known to and understood by me. I understand this consent form and agree to its conditions on behalf of my child.
